OWNER SURRENDER FORM

Please answer the following questions about the dog you need to surrender.  HART is a 100% foster based organization and the only way we can take in a dog is if we have a foster home for it to go to.  Filling out this form in no way guarantees we can take your pup but helps us ask for a foster.

Is the dog up to date on shots?
Has he/she been spayed/neutered?
Does he/she have a microchip?
Have you taken the dog to the vet?

If yes please provide your veterinarian information

If Yes, Permission to contact your vet? (many vets now require permission from client directly, please let them know we will be calling)

SKILLS TEMPERAMENT

Is the dog good with children?
Good with other dogs?
Good with cats?
Is the dog potty trained?
Have you taken the dog to any obedience training?
Has the dog ever bit a human?
Have you had any behavioral issues with the dog (many behavioral issues can be worked on but we need to know to place the dog properly)?

BASIC INFO

FINANCIAL RESPONSIBILITY

All dogs must be up to date on vetting and spayed/neutered before we can list a dog up for adoption. If the dog is not currently up to date on vetting and spayed/neutered are you willing/able to pay the vet costs?
There is a fee associated with relinquishing a dog into HARTS care made to HART in the form of a donation. Are you prepared to make a monetary donation to HART?

By submitting this form you agree to allow HART to share the dog(s) information with our Dog Foster Community in an attempt to find a foster for the dog in your possession.  If we find a foster willing to take the dog the foster will then be given your personal information to make contact for surrender.  If interested in fostering for HART please fill out the dog foster application next.
